Tuberculosis pancreática: una causa insospechada de dolor abdominal y fiebre

RESUMEN
Introducción: Si bien con frecuencia la tuberculosis es la forma como se manifiesta la infección por el virus de la inmunodeficiencia humana (VIH), incluso en pacientes que no han desarrollado síndrome de inmunodeficiencia adquirida, la tuberculosis pancreática es poco común y clínica y radiológicamente puede parecer carcinoma pancreático.
Caso clínico: Hombre de 42 años de edad con dolor abdominal de cinco días de evolución en cuadrante inferior derecho, fiebre y vómito sin diarrea; no tenía antecedentes de cirugías abdominales. La tomografía axial computarizada reveló una estructura multiquística en la cabeza del páncreas. Se realizó pancreaticoduodenectomía debido a sospecha de malignidad y a que las biopsias por punción fueron inadecuadas para el diagnóstico; el análisis histopatológico indicó tuberculosis pancreática. El paciente fue dado de alta al décimo día posoperatorio sin complicaciones quirúrgicas; 10 meses después murió a causa de neumonía por
Pneumocystis jirovecii. La determinación de anticuerpos VIH fue positiva.
Conclusiones: Por lo general el diagnóstico de tuberculosis del páncreas no se realiza antes de una laparotomía, a menos que haya evidencia de tuberculosis pulmonar. La tuberculosis del páncreas debe ser considerada en el diagnóstico diferencial de una masa en la cabeza del páncreas. La respuesta al tratamiento antituberculoso temprano es muy buena.
