Incidencia de las alteraciones microcirculatorias en pacientes con sepsis severa que ingresan a la Unidad de Cuidados Intensivos del Hospital General “Ignacio Zaragoza”

RESUMEN
El estudio tiene el propósito de identificar las alteraciones microcirculatorias en pacientes con sepsis severa en la Unidad de Cuidados Intensivos del Hospital Regional General “Ignacio Zaragoza” ISSSTE; se evaluaron pacientes que ingresaron del 1 abril de 2007 al 30 abril de 2008, entre 18 y 70 años; se excluyeron pacientes con enfermedad pulmonar obstructiva crónica, con apoyo de norepinefrina o dopamina a dosis alfa y con paro cardiaco. Estudio observacional, prospectivo, descriptivo, abierto y longitudinal. 35 pacientes, 18 hombres y 17 mujeres, promedio de 57.8 años, 72.62 kg y 1.65 m. Se calculó el consumo de oxígeno arterial, venoso y capilar, diferencia arteriovenosa de oxígeno, índice de extracción de oxígeno, transporte de oxígeno y consumo de oxígeno en las primeras 24 h de diagnóstico. Se mostraron alteraciones con significancia estadística p ‹ 0.05, excepto el índice de extracción de oxígeno, transporte y consumo de oxígeno. Los resultados corresponden a la bibliografía, en los cuales los cambios en la microcirculación se reflejan antes de las 24 h con alteraciones significativas, caracterizadas por incremento del gasto cardiaco como forma compensadora, disminución de la extracción de oxígeno, disminución del consumo de oxígeno y disminución de la diferencia arteriovenosa de oxígeno.
