Virus del papiloma humano. Actualización y presentación de un caso de carcinoma esofágico asociado a VPH

RESUMEN
Se hizo una amplia revisión bibliográfica en relación a los virus del papiloma humano (VPH) a propósito de la presencia de imágenes coilocíticas en extendidos citológicos de cánceres de esófago. En nuestro caso se realizó citología por cepillado y biopsia a un paciente con epigastralgia y disfagia marcada a los sólidos; endoscópicamente se diagnosticó esofagitis grado IV con ulceraciones y pseudopólipos en la unión esofago-gástrica. El extendido citológico mostró abundantes células típicas del VPH (coilocitos) con núcleos malignos. La enfermedad evolucionó violentamente y en las últimas endoscopias la tumoración ocluía la luz esofágica, pero las biopsias no eran concluyentes de malignidad. El paciente fue operado y el estudio histopatológico de la pieza quirúrgica demostró carcinoma epidermoide bien diferenciado de esófago, de aspecto verrucoso de baja malignidad por VPH.
