Empleo de la prueba de esfuerzo Bruce en una población de riesgo asintomática

RESUMEN
Objetivo: Evaluar la utilidad de la prueba de esfuerzo para identificar la enfermedad arterial coronaria en personas con factores de riesgo, asintomáticos.
Material y métodos: Estudio transversal comparativo, en hombres ≥ 40 años o mujeres postmenopáusicas, sin síntomas cardiovasculares, con factores de riesgo coronario, sin contraindicación para la realización de prueba de esfuerzo o para el cateterismo cardiaco. En todos se realizó prueba de esfuerzo Bruce y coronariografía selectiva. Análisis estadístico: Razón de momios e intervalos de confianza. Correlación de Spearman (p ‹ 0.05) y al análisis Bayesiano, para la utilidad de la prueba.
Resultados: Estudiamos 18 sujetos: ocho mujeres y diez hombres, con edad 58.5 ± 9.3 años. Diez con pruebas de esfuerzo positivas, el resto negativas; 12 con lesiones coronarias. La asociación de hipertensión, tabaquismo e hipercolesterolemia arrojó una razón de momios de 7 para la probabilidad de lesiones coronarias. El coeficiente de correlación entre los resultados de la prueba de esfuerzo y los hallazgos del cateterismo es de r = 0.316 (p ‹ 0.05). La sensibilidad de la prueba es de 0.66, especificidad 0.66, valor predictivo positivo 0.80 y negativo 0.50.
Conclusión. El rendimiento de la prueba es satisfactorio para detectar cardiopatía isquémica silente en sujetos asintomáticos.
