Sarcomas intraescrotales. Estudio de 13 casos

RESUMEN

Introducción: Los sarcomas intraescrotales paratesticulares y primarios testiculares son tumores malignos mesenquimatosos poco frecuentes del aparato genitourinario masculino. Objetivos: Identificar los sarcomas intraescrotales detectados en el Hospital General de México en un periodo de trece años. Clasificar las variedades histológicas. Comparar los resultados con los descritos en la literatura. Material y métodos: Se realizó la búsqueda de sarcomas intraescrotales en los archivos de la Unidad de Patología del Hospital General de México de 1995 a 2007. Se clasificaron histopatológicamente y se realizaron estudios de inmunohistoquímica para confirmación diagnóstica en los casos que lo requirieron. Resultados: Se encontraron 13 sarcomas, ocho rabdomiosarcomas, tres liposarcomas, un leiomiosarcoma y un mesenquimoma maligno. Cuatro rabdomiosarcomas, dos liposarcomas y el mesenquimoma maligno fueron paratesticulares. Un rabdomiosarcoma se originó en neoplasia germinal y en cinco sarcomas no fue posible identificar el origen anatómico. Conclusiones: Los liposarcomas son lesiones paratesticulares y aparentemente no tienen relación con neoplasias germinales. El rabdomiosarcoma puede ser predominantemente paratesticular o formar parte de una lesión germinal, lo que determina un comportamiento clínico distinto, así como variación en el tratamiento; en algunos casos no es posible definir el sitio de origen debido a la falta de muestreo extenso que permita la identificación de tejido testicular residual.
