RESUMEN
La sepsis y el síndrome de insuficiencia respiratoria aguda (SIRA) representan dos grandes retos terapéuticos para el intensivista. En los últimos años y en base a los avances en el conocimiento de la fisiopatología de estas dos entidades se han diseñado estrategias terapéuticas y guías de recomendación con el objetivo de unificar criterios e incrementar la sobrevida. En este trabajo se presentan dos casos de sepsis grave de diferente etiología que desarrollaron SIRA en sus variantes pulmonar y extrapulmonar, con la finalidad de dar a conocer a los clínicos la diferencia entre estas dos entidades y los principios de manejo basados en la evidencia científica con los que fundamentamos el tratamiento.
