Comparación de la función diastólica por Doppler tisular en pacientes con cardiopatía isquémica pre y postangioplastia

RESUMEN
Objetivo: Comparar la función diastólica ventricular izquierda mediante ecocardiograma Doppler tisular (DTI) en el anillo mitral y ecocardiograma Doppler convencional (EDC) en pacientes con cardiopatía isquémica (CI) antes y después de la angioplastia coronaria transluminal percutánea (ACTP).
Material y métodos: Estudio observacional, comparativo, longitudinal y descriptivo (agosto 2007- enero 2008), sobre los beneficios de la ACTP en la función diastólica en pacientes isquémicos, valorado por EDC y EDT preangioplastia y 2 meses postangioplastia.
Resultados: Se estudiaron 32 pacientes, 28 hombres (84%) y 4 mujeres (16%), edad promedio 58.8 años, con diagnóstico de CI (8 infartos y 26 angina ), con EDT encontrando índice E/A 0.77, y por EDC índice E/A 1.05, 2 meses post-ACTP por EDC índice E/A 1.08 y por EDT índice E/A 0.82, con mejoría posterior por ambos (p ‹ 0.05). Nueve (22%) de los 32 pacientes, a 2 meses se encuentran sin disfunción diastólica índice E/A 1.15.
Conclusiones: La función diastólica del ventrículo izquierdo medida por EDT tiene mayor sensibilidad y especificidad en relación con el EDC.
