Insuficiencia venosa crónica, correlación clínica y ultrasonográfica

RESUMEN
Objetivo: Comprobar el porcentaje de pacientes con insuficiencia venosa crónica (IVC) que presentan patrones de reflujo valvular en algún segmento del sistema venoso.
Métodos: Se realizó un estudio prospectivo, observacional y transversal en el Hospital Lic. Adolfo López Mateos en el periodo comprendido entre el 1 abril y el 31 de julio de 2004. Se incluyeron mayores de 18 y menores de 65 años; excluyéndose a aquéllos con patología arterial y neuropatía periférica. Se incluyeron pacientes con diagnóstico de IVC, evaluándose con la Escala Clínica de la Severidad Venosa (ECSV) y efectuándose ultrasonido Doppler dúplex (USDDC) de las extremidades inferiores, analizándose en total ocho segmentos venosos para valorar presencia de reflujo patológico. 2.
Resultados: Se evaluaron un total de 47 pacientes y 51 extremidades inferiores por USDDC. El dolor fue incapacitante en 19.6% (10 pacientes) y no limitante en 39.2% (20 pacientes). Las várices fueron con mayor frecuencia de escala moderada (51%) y 31.4% de severas. El análisis fue estadísticamente significativo en el grupo de la VSI alta y baja con un valor de p = 0.029 y 0.006, respectivamente. Corroborando la relación entre reflujo y grado de las várices en estos dos segmentos.
Discusión: La cirugía venosa es un tratamiento efectivo para la IVC cuando el reflujo está confinado al sistema venoso superficial, la habilidad para establecer patrones de mapeo para la evaluación del reflujo con medios ultrasonográficos es crucial para un adecuado manejo de la enfermedad venosa.
