RESUMEN

Propósito: Mostrar el papel de la laparoscopia en el diagnóstico y estadificación en las enfermedades malignas.
Obtención de la información: Consulta y revisión de la literatura, 81 referencias.
Selección de los estudios: Análisis de todos los artículos en relación al uso de la laparoscopía con fines diagnósticos o de estadificación en las enfermedades malignas abdominales, incluyendo su desarrollo histórico, indicaciones, técnicas, algoritmos y secuencias a seguir para los cánceres más frecuentes.
Extracción de los datos: Sintetizando los datos específicos de cada tipo de cáncer en las series con mayor casuística y las ventajas de realizar un diagnóstico y estadificación adecuada, para el manejo óptimo de estos enfermos.
Resultados: El papel de la laparoscopia en las enfermedades malignas había sido limitado solamente al diagnóstico, en los últimos años su utilización para la estadificación ha tenido un gran auge con buenos resultados. La utilización del lavado peritoneal, la obtención de biopsias y el ultrasonido translaparoscópico han mejorado la precisión en la evaluación de las enfermedades malignas y como consecuencia, evitar laparotomías innecesarias, menor trauma quirúrgico, permitiendo incluso el realizar tratamientos paliativos por laparoscopia en el mismo tiempo quirúrgico. El manejo óptimo del cáncer requiere de una estadificación segura facilitando la terapia adecuada del enfermo oncológico.
Conclusión: La laparoscopia debe ser utilizada para el diagnóstico y estadificación de las enfermedades malignas, tiene un impacto significativo en la terapéutica quirúrgica debido a que sus hallazgos modifican el estadio y, como consecuencia de esto, determinan el manejo óptimo de los padecimientos oncológicos.
