RESUMEN
Los aneurismas de la porción intracavernosa de la arteria carótida coexistentes con fístula carotido-cavernosa (FCC) no son comunes en la literatura. Cuando se rompen causan una FCC más que sangrado en el espacio subaracnoideo. El tratamiento de elección es el abordaje endovascular.
Caso clínico: Masculino de 65 años que desarrolla cefalea bitemporal en un periodo de 24 horas, la angiografía por resonancia magnética y la angiografía con substracción digital mostraron un aneurisma gigante no roto de la arteria carótida intracavernosa izquierda con una fístula que comunicaba el aneurisma con el seno cavernoso del lado derecho, con extensión del aneurisma dentro del seno esfenoidal. Basados en la clasificación de Barrow, este caso se describe como una fístula tipo A, este tipo es usualmente rara, tiene flujo alto y está asociada a un evento traumático; en este paciente su origen fue espontáneo. En la presentación de este caso, se muestran los hallazgos con resonancia magnética, angiorresonancia y angiografía cerebral con substracción digital, así como una breve revisión de la literatura. En nuestro conocimiento, la coexistencia de un aneurisma cavernoso no roto con una FCC conectada al seno cavernoso contralateral, ha sido rara vez reportada.
