Mecanismos inmunomoduladores de la gammaglobulina intravenosa (IGIV) en alteraciones del complemento

RESUMEN
El complemento es parte del sistema inmune innato, con dos actividades importantes, la defensa contra infecciones y procesos inflamatorios. Éste actúa conjuntamente con el sistema humoral, para la eliminación física de los patógenos y la inflamación no específica. Alteraciones en varios componentes del complemento han sido ya asociadas con una amplia variedad de padecimientos como infecciones recurrentes, angioedema o autoinmunes. Se han propuesto varios mecanismos de acción de IGIV. Estos incluyen bloqueo de los receptores FC, aceleración de la limpieza de autoanticuerpos patógenos endógenos, inhibición de componentes de la cascada del complemento, unión, interferencia de la activación o estimulación de la inactivación de componentes del complemento, lo cual previene daño tisular, también inhibe y absorbe C3 y C4 en las células y tejidos blanco. La IGIV tiene también efectos anti-inflamatorios que previenen la generación del complejo de ataque de membrana, evitando deposición de estos fragmentos en la superficie de células blanco con el consecuente daño tisular. El propósito de este artículo es explicar los posibles mecanismos de acción de la IGIV en desregulaciones del complemento, lo cual dará más opciones terapéuticas en enfermedades con alteraciones del complemento.
