COVID-19, enfermedad de Kawasaki y síndromes inflamatorios multisistémicos: una conceptualización desde los principales reportes

RESUMEN
Objetivo: Describir las características clínicas de pacientes con COVID-19 y una
variedad atípica de la enfermedad de Kawasaki con procesos inflamatorios sistémicos
en niños y sus procedimientos diagnósticos y terapéuticos.
Metodología: Revisión bibliográfica narrativa, a partir de la consulta en las bases
de datos de: PubMed, Science Direct y Google académico. Para la búsqueda se utilizaron
las palabras clave (MeSH): “COVID-19”, “Kawasaki Disease”, “SARS-CoV-2”,
“multisystem inflammatory syndrome”. Se incluyeron artículos originales y reportes
de casos clínicos.
Resultados: Se encontraron 52 artículos, de ellos se seleccionaron 27 que reunieron
los criterios de inclusión para su análisis.
Conclusiones: Con las manifestaciones clínicas asociadas con la COVID-19
aparecieron cuadros inflamatorios multisistémicos atípicos y la enfermedad de
Kawasaki establece una urgencia de datos precisos y prospectivos de este síndrome
que logre diferenciar, en su totalidad, la de Kawasaki o, en su defecto, definirla como
una variante de la misma. Los médicos a cargo de pacientes con COVID-19 deben
conocer los criterios diagnósticos del nuevo síndrome y estar alertas, sobre todo en
países con número de casos constantes donde podría verse la mayoría de pacientes
complicados luego de la infección por SARS-CoV-2. Además, estas actualizaciones
de la investigación implican una perspectiva aún más profunda para el estudio
fisiopatológico, la indicación del tratamiento y la conceptualización de las bases
clínicas para su entendimiento.
